To qualify for assistance, you must be a resident of Stokes County and at least 18 years of age. You must complete this application in its entirety, SUBMIT PROOF OF INCOME, and email all information to us at: [email protected]. *CHECK THE INCOME GUIDELINES BEFORE COMPLETING AN APPLICATION. Completing an application when you do not qualify takes up valuable time and resources for our volunteers* Once your application is approved, you will be contacted via email. The pet that is approved has to have proof of a current rabies vaccine. If your pet has not been vaccinated, the rabies vaccine will be given at the time of surgery AT THE OWNERS EXPENSE. State law requires a current rabies vaccination for any dog or cat four months or older. You will also be responsible for transporting the animal to and from the facility providing the spay/neuter surgery. Click the button at the bottom of the page to apply if you qualify. If you do not qualify through our program, there are other resources for spay and neuter- https://humanesolution.org is another resource (now called Spay Kind). They offer vouchers for dogs, cats and feral cats.
We will assist you in having your companion pet spayed or neutered if you meet the below income guidelines. You will be asked to pay a $20 copay for cats and a $30 copay for dogs. Spay Stokes would pay for the remainder of the surgery costs that involve only the spay or neuter and pain medication. Any other issues found at the time of surgery will be the owner's responsibility to pay. Your pet must have a current rabies vaccination or you will also have to pay $15 for a rabies vaccination) DUE TO LIMITED FUNDING, NO MORE THAN TWO DOGS CAN BE SPAYED OR NEUTERED PER HOUSEHOLD (LIFETIME) . We can possibly assist with more than 2 cats per property if funds allow and this can change on a weekly basis. List ALL animals you own at the time of your application. If you add a pet after you apply, just contact us to let us know (please do not complete more than one application in the same year from January- December) You will need to complete an application either online or one can be mailed to you (be aware that this is a much slower process). You must be 18 years of age to complete an application. CHECK THE INCOME GUIDELINES BELOW BEFORE COMPLETING AN APPLICATION. Applications CANNOT be processed without proof of income (check stub, W-2. tax return, etc.) This information can be emailed to us at: [email protected]. *Once you submit an application, we will contact you via email to let you know we have received it. If you do not have an email address, we will call or text you. If we do not hear back from you after a period of time we will call or text you to follow up. Once a follow up phone call or text is made, it is your responsibility to provide the information we request in order to process your application. Failure to provide the necessary proof of income after two months will result in your application being placed into our inactive file. Applications missing information will not be processed. Spay Stokes has the right to refuse financial assistance for any reason.
** If you are applying for financial assistance, please DO NOT call Central Spay and Neuter and schedule an appointment. If you qualify for assistance, we will schedule your appointment for you. We can deny your request for assistance if you have already scheduled an appointment prior to completing our application. **
